Eligibility
Open to birthing persons and infants less than 6 months of age with risk factors which may include:
  • Income eligibility
  • Diseases that affect pregnancy or infants
  • Homelessness or in unstable housing
  • Teen or over 45 years old
  • Previous pre-term delivery or low birth weight
  • Mental health issues
  • Previously incarcerated or under house arrest
  • DCFS involvement in the past year
  • Victim of domestic violence
  • Substance abuse during pregnancy
  • Complex medical issues

Provides one-on-one support throughout pregnancy.

Offers personalized support for a healthy pregnancy, full term delivery, and postpartum care through the baby’s first 6 months.

Navigators (who are nurses and social workers) can also connect clients with community resources and help find medical care.

Those who qualify for the program may receive:

- Personalized prenatal and postpartum education.

- Community referrals and links to services.

- Services provided free of charge.
