Ages 55+; Must be certified by the state to need nursing home care; Must be able to live safely in the community at the time of enrollment, and be living in one of the following PACE Zip codes: 60453, 60455, 60456, 60458, 60459, 60501, 60609, 60629,60632, 60636, 60638, 60652

Application process
Interested individuals are invited to visit the PACE center. During the visit, they can explore the space, meet the care team, and ask questions. They will talk with individuals about their health and support needs. Contact (773) 819-7723 to schedule a visit.
