Application process
(1) Call 630-718-0717 ext. 240 for the intake interview and to schedule on-site appointment. (2) Complete the Welcome Package before appointment. Welcome Package can be emailed to the applicant or is available in the lobby on site to be completed before appointment. (3) Bring all required documentation. (4) Guidelines for Day of Appointment: (a) Children under the age of 11 not to be left by themselves in the lobby, (b) Parents must be present on-site with their children during sessions.
