Application process
(1) If a Mental Health provider has not seen you in the last 12 months, you must first complete a Mental Health Intake (2) Ask your primary Mental Health provider (or the Mental Health Intake Center) to place a consult for CWT (3) You will need to be medically cleared by a healthcare provider to participate; (4) Once your consult is received, CWT will conduct a thorough review of your records to determine the most appropriate program (5) You will then be contacted and invited to participate in a CWT intake interview and develop a treatment plan that reflects your individualized goals toward employment

Specialized programs include Blind Rehabilitation, Spinal Cord Injury, Neurosurgery, Radiation Therapy and Cardiovascular Surgery. Operates a food pantry, provides COVID-19 vaccines and provides information and referral to eligible Veterans. Also offers the compensated work therapy program for Veterans.
**Pantry will be closed on 12/25/25 and 01/01/26, and will resume normal operations on 01/08/26**
