You can request an Intake Packet to get started or call the office for more information. Only individuals who complete the full intake form and provide the requested documentation will be counseled. Incomplete forms and documentation will postpone their ability to assist you. If you need help with completing your paperwork or accommodations, please get in touch with the office.
