Must reside in Addison Township; Must be 18 years of age and out of High School; Must meet income guidelines.
Required documents
Photo ID (Drivers License, State ID, Passport)
Social Security cards of all family members or ITIN (Individual Taxpayer Identification Numbers
Proof of income for all household members for the 30 days prior to the appointment (such as pay stubs)
Proof of Residence (copy of current utility bill, rental lease showing utilities are included, mortgage statement).
