Service Animals | Canine Companions

Eligibility
People with physical or developmental disabilities who can demonstrate that an assistance dog will enhance their independence or their quality of life; At least 18 years old with an established, stable home life; Children must be at least 5 years old to be considered for a skilled companion

Application process
(1) Complete the online application request form or call 1-800-BARK to receive one by mail (2) An application packet will be mailed to you within 4-6 weeks (3) Complete and return the application to the training center indicated (4) A telephone interview may be scheduled (5) If appropriate, medical and/or professional reference forms must be filled out (6) A personal interview may be scheduled (7) The review committee will determine acceptance (8) Training dog at center can take 5 to 9 months
