Inpatient Services: 4 years of age and above; Partial Hospitalization: Children 4-11 years, adolescents 12-17 years, and adults 18 years and above; Intensive Outpatient: Children 4-11 years, adolescents 12-17 years, and those who are 18 years of age and attending high school.

Application process
(1) Call (630) 540-3700 to contact Assessment and Referral; (2) Clinicians will assess the crisis level and suggest the right services; (3) If needed, an intake assessment will be recommended.

Medicare (inpatient only), Medicaid, Self-Pay, Private Insurance.
