Must meet income guidelines - married, filing jointly or head of household: $65,000 or less, for all other filings, must be at or below $30,000

Valid photo ID, Copies of all W-2 ,1099 tax forms and all tax statements you have received Information for other income, Forms 1095A, 1095-B or 1095-C, Information on all items you wish to deduct, Social Security Cards for all individuals who will be listed on the tax return, Birthdates for all people listed on the return, A copy of last year's tax return, Total paid to daycare provider and their tax ID, Proof of health insurance (if applicable), Address and Tax ID for any child care providers, Bank routing and account numbers or direct deposits
