- Copy of Illinois Disabled Person Identification Card (PDF); or
- Proof of Social Security Administration Social Security Benefits (Award letter, verification letter, or annual cost of living adjustment); or
- Proof of Veterans Administration disability benefits; or
- Proof of Railroad or Civil Service disability benefits
