Community Reintegration Program | Lake County Center for Independent Living

Assists individuals with disabilities who are currently residing in a nursing home and want to move to their own home. Services include:

- Information on accessible and affordable housing options.

- Financial assistance with the cost of moving back into the community.

- Assistance in locating personal assistants.

- Independent living skills training.

- Information and referrals to community agencies and other support services.

- Information on assistive technology and devices.

- Determining eligibility for DRS Home Services.

- Advocacy.

- Benefits counseling and referral.

- Peer counseling.
