For Weekday Medical Transportation:
- No application required
- Must call at least three days in advance to schedule a ride
- Visit in-person or online for program application
- Must call by Friday to schedule weekend ride/s
- Visit in-person or online for program application
- A debit card will be issued by mail and automatically loaded with $18.00 on the first of each month
- Photo ID
- Current utility bill with name and address

Provides curb to curb transportation for medical appointments within the township. Rides must be requested in advance. Shuttle runs weekdays only and is equipped with a wheelchair lift.
Weekend Transportation
Provides curb to curb transportation on weekends for travel within 10 miles. Rides must be requested in advance. Walking aids must be able to fold down for storage.
Transit Card
Issues a debit card to be used for personal transportation services. Funds can be used for any type of transportation service including taxis and ride shares.
