The AIDS Foundation of Chicago's case management system offers a wide array of services, functioning as a connector between clients and resources. Its system identifies medical and social service needs and facilitates referrals to address those needs; helps clients apply for benefits, including housing, transportation, food, and nutrition services; and enables access to emergency financial assistance during crises. AFC has also made significant investments in a network of providers that offer intensive case management and supportive housing options for people living with various chronic illnesses.
