Prescription Drug Patient Assistance Programs | Pfizer Rx Pathways

Eligibility
Must have a valid prescription for a Pfizer medicine for an approved use; must have income at or below 300% of the federal poverty level; must live in the United States or a U.S. territory; must be treated by a licensed U.S. healthcare provider; must be uninsured or have government insurance and cannot afford the medication; cannot have commercial (private) insurance; must show that they cannot afford their prescription costs; may need to apply for other assistance programs first and be denied; for Medicare Part D or Advantage, must enroll in the Medicare Prescription Payment Plan and not have reached the yearly out-of-pocket limit

Application process
(1) Complete and sign the application form. (2) Have the prescribing healthcare provider complete their section of the form. (3) Include a valid prescription for the Pfizer medication. (4) Submit the application by fax or mail as listed on the form or apply online if available. (5) Wait for the program to review the application and respond to any requests for more information. (6) If approved, medication is shipped or provided as directed by the program.
