Eligibility
Under 60 years old, living with severe disabilities lasting at least 12 months; Be at imminent risk of nursing facility placement; Meet Medicaid eligibility;
Needed services cost less than nursing home; Proof of legal residency or U.S. citizenship

Application process
1) Call or go online and ask to refer self or someone else for services
(2) Staff will take down information and entered into the case management system
(3) Staff will be assigned to visit applicant at home to discuss eligibility and available services and complete application
(4) Staff will obtain all needed or releases of information, and copies of Photo ID
(5) A service plan will be developed and applicant will be assisted in locating in-home service providers
