Eligibility
The passenger must have a financial need
- Be able to fly in a non-pressurized airplane
- Be ambulatory and sit upright in a standard aircraft seat
- Be responsible for their own medications
- Not have a communicable disease
- Provide DOT-approved supplemental oxygen (if needed)
- Provide their own medical assistance (if needed)
- Provide own ground transportation to and from airports
- Be able to board small airplanes unassisted.
Required documents
Phone/fax numbers of physicians so a medical release can be obtained (will contact the physician to verify ability to fly safely, as well as discuss the passenger's situation with a 3rd party such as a minister, social worker, nurse, etc.
- Sign a liability release form and provide a photo ID (Driver's License)

Application process
Log on to AFC website (www.angelflightcentral.org), click on "Passengers" and then click on "Request Assistance." complete online questionnaire and click "submit.", information will immediately be sent to a Flight Coordinator who will begin the qualification process
- Telephone to apply if do not have access to a computer
- Referral from social workers, nurses, physicians, treatment facilities are helpful, but not required
