Eligibility
Must be a resident of DuPage County for at least 30 days and intend to live in DuPage County year-round; Must not be eligible for any other type of health care coverage including Medicaid, Medicare, ACA health insurance, and employer-sponsored insurance; Must be at least 19 years of age; Household incomes must be at or below 250% of the current federal poverty level (FPL).
Required documents
Photo ID (passport, state ID, driver's license) Proof of income (paystubs, W-2 forms, Social Security awards letter or pension) Proof of address (utility bill issued within last 60 days, driver's license or state ID issued within last 30 days, current lease or proof of current mortgage).
