Must be a resident of Illinois; Must frequently be in the hospital, or be living with a chronic illness covered by the program, or must be experiencing a sudden or gradual decline in health, or must have side effects from some other medical treatment.

(1) Call during posted hours to make a referral or inquire about receiving services (2) The office will check insurance, eligibility, and benefits (3) The palliative team will schedule an initial visit with the patient and their family.
