- Pediatric Dentistry: Ages 2-21 years Medicaid recipients
- Special Needs Dentistry: Children with Down syndrome, cerebral palsy, autism, ADHD, genetic syndromes or other disorders

- List of medications currently taking
- Dental records from previous dentist (if transferring)
