Eligibility
Must be living in Illinois at the time of filing of application; Must be 65 years of age or older before January 1 of current year, OR; Must become 65 years of age in the current year, OR; Must be 16 years of age or older before January 1 of current year and be determined to be totally and permanently disabled as defined by program requirements; Must meet household income guidelines.
Required documents
Photo ID (driver's license, state ID) Proof of residence (driver's license, state ID, utility bill with name of applicant, mortgage or rent statement) Proof of income (W-2 forms, tax documents).
