Application process
(1) Visit www.GuideDog.org for more information on the program (2) Click on the "Apply Now" button from the "Get a Guide Dog" page to apply online (3) The application can also be printed and faxed to number provided (4) Call 631-930-9070 to have a team member help you with the online application (5) Braille applications are available on request; (6) Once the application is submitted, a field service representative will follow up with each applicant within 5 days to conduct a phone or in-person interview (7) Submit or bring required documents (8) Attend in-person training with matched dog, which will need 9-18 months of training before matching with client
