Eligibility
Eligible patients must be enrolled members of a U.S. federally recognized tribe and possess a tribal ID or Certificate of Indian Blood. Prescriptions must be filled at one of AIHSC's registered Walgreens 340B pharmacies. The patient must have active health records with AIHSC.

Contact (773) 883-9100 to establish care or set up an annual visit to AIHSC. Bring AIHSC Provider Voucher(s) when picking up prescriptions at one of the Walgreens 340B Pharmacy Locations.
