Must be at least 19 years old; Minors who are 17 or 18 years old may apply with parental or legal guardian consent (unless an emancipated 17 or 18 year old minor; emancipated minors are required to provide court order declaring they are emancipated)

Proof of identity/age: valid driver's license, passport, or birth certificate If a birth certificate is used it must be a certified copy If proof is not furnished, the license cannot be issued Contact office for questions concerning identification papers
