Must fulfill one of the following: Resident of Elmhurst for at least 90 days; Children enrolled in Elmhurst public schools; Member of a participating church.
Required documents
2 proofs of 90 day residence in Elmhurst (driver's license, state ID, utility bill or lease) Proof of income for all household members Letter from pastor confirming church members (if not an Elmhurst resident) Report card or a letter from your child's school social worker (if not an Elmhurst resident).
