AIDS Foundation of Chicago's corrections case management program assists HIV-positive detainees as they are released from incarceration. The program begins supporting clients the moment they leave jail or prison. With help from AFC and other partners, these clients receive initial medical care, housing assistance, substance abuse treatment, and mental health services.
