Firman Community Services offers a Family Case Management (FCM) program that provides comprehensive service coordination to improve the health, social, education, and developmental needs of pregnant women, and infants (0-12 months) from low-income families in the communities of Illinois. The program connects an individual with a medical provider while helping one to understand the importance of regularly scheduled well-child visits and immunizations. Other resources the program services can connect an individual to are family planning, WIC, health education, hearing/vision, substance use support, parental/parenting classes, child care, housing, smoking cessation, lead screening, transportation, early intervention, immunizations, well-child exams, All Kids (health insurance) and domestic violence support.
