To become a CDAC provider: must be 18 years of age or older, a family member, friend, or neighbor of the person you are helping (cannot be a spouse or parent/steparent to the person you are helping), have the training or experience to provide the service and pass an abuse and criminal background check

Provides a program that allows Home and Community Based Services (HCBS) Medicaid waiver program recipients the option of finding and hiring their own in home care. CDAC in home care providers can offer either skilled services (medical in nature) or unskilled services (normal daily life activity). All CDAC Agreements must be approved by the Medicaid members case manager or Department of Human Services service worker. TO BECOME A CDAC PROVIDER - contact the Iowa Medicaid Enterprise and request an application to become an individual HCBS CDAC provider (Provider Enrollment Department). The IME will mail an application form with instructions for filling it out and request a background check to be done. Send in completely filled out application to IME Provider Enrollment, PO Box 36450, Des Moines, IA 50315.
