65 years or older or a person with a disability; Qualifying military personnel living in the service region;
Required documents
Valid Photo ID
Wallet-sized color photo of yourself/applicant that is about 2 inches by 2 inches
If you are a person with a disability, proof of the disability (i.e. Social Security Disability payments, Veteran's Administration letter, etc.)
If you are receiving Medicare, verification of Medicare status.

Application process
(1) To apply in person, call the main number to find a location nearest you. (2) To apply by mail, download either the Senior or Persons with Disabilities application from the website and return the completed application to the address as listed on the application https://fares.rtachicago.org/. (3) Provide required documentation. (4) If approved, a permit will be received by mail in about 3-4 weeks.
Fees
No Fees, Fees apply for replacement Permits or for pictures to be take on site.
