Helps provide a continuum of care for persons living with HIV/AIDS. Clients in the program will be assigned a medical case manager. Case managers can assist clients by linking them with a network of health care and support services. Services may include:

  • Rental/mortgage assistance
  • Utility assistance
  • Permanent supportive housing
  • Medication programs
  • Medical benefits assistance
  • Vision services assistance
  • Dental services assistance
  • Legal services assistance

  • Mental health services assistance
  • Substance mis-use services
