Offers assistance filing for legal name change in the State of Illinois for LGBTQ+ individuals. Offers assistance with the following:

• Providing necessary court forms for the 19th Judicial Circuit Court
• E-filing court forms
• Providing name change and gender designation forms for the Illinois Department of Motor Vehicles, Social Security Administration, Illinois Department of Public Health and other states where available
• Obtaining certified copies of court orders related to name changes
• Alternate mailing address for court forms and notices
• Financial assistance with paying for e-filing costs (for those who qualify)
• Courtroom advocacy / accompaniment services

TJLP utilizes paid staff and volunteers to conduct in person name change clinics and virtual 1:1 name change support and provides assistance to petitioners to update their name and gender marker on various identity documents (birth certificate, passports, drivers license and ID and other miscellaneous documents).
