Serves seniors with the goal of helping them remain safe and in their homes for as long as possible. The program focuses on chronic disease prevention, nutrition, and physical activity. Department nurses offer low-cost foot care and blood pressure screenings, home visitations, and educational presentations. Staff works with community partners, reaching most program participants through senior centers, churches, and independent senior living communities.
Provides general medical care services. Including blood pressure screenings, breastfeeding peer counseling, car safety seat program, child and adult immunizations, developmental screenings, family case management, health screenings, Illinois tobacco-free communities' program, lead poisoning prevention program, WIC program, lice education, nutrition education, pregnancy testing, TB screenings and tests, vision and hearing screenings, and the women's health clinic.
