Adult wellness clinic offers full lipid profile (Cholesterol) with glucose, tuberculosis clinic, and immunization clinic.

Primary medical care services are available for most minor or intermediate illnesses.
Additional services include adult physicals, community wellness groups, and health screenings targeting individuals at high risk for conditions such as high blood pressure, diabetes, high cholesterol, or cancer. Prescription assistance is provided to support medication needs. The program also offers nutrition education, which includes guidance on healthful eating, food handling, preparation, shopping skills, and the relationship between nutrition and overall health. Information and counseling about Affordable Care Act (ACA) insurance options are available, including assistance with enrollment in health insurance plans through the marketplaces, facilitated by trained navigators and application counselors. All services are open to individuals regardless of background or circumstances.
Provides information and services related to health and wellness topics, including heart health, cholesterol, blood pressure screening, smoking cessation, diabetes, obesity, and physical activity. Offers blood pressure screenings to identify individuals at risk for hypertension-related conditions, cholesterol and triglycerides tests to assess cardiovascular health, and hypoglycemia screenings to detect blood sugar deficiencies. Additionally, provides diabetes management support, including education on self-management skills, dietary planning, exercise, and prevention of complications. Health education presentations are also available for local schools.

--Immunizations are available for persons of all ages. Red Willow County Health Department accepts private insurance and also has programs for free immunizations for individuals without insurance.
--Lab draws are every Thursday morning by appointment. Panel $50.00, PSA $35.00, TSH $30.00, HA1C $25.00, and Vit D $55.00.

Community based health center offering adult and pediatric primary care. Provides a medical home for Chestnut clients, especially those with co-morbid mental illness and/or substance use disorders. Services include:
- Basic primary care, preventive care, and prescription services.
- Screenings (cancer, communicable diseases, cholesterol, blood lead, pediatric vision, hearing, and dental).
- Annual wellness exams for all ages.
- Well child exams and immunizations.
- Lab collections.
- Obstetrical and gynecological care.
- Physicals (school, sports, employment).
Health center for care of acute and chronic illness. Transportation available for appointments at the clinic. Provides community services like screening clinics for blood pressure, blood sugar, cholesterol, breast and cervical cancer programs, food clinics (foot screenings and care), disease investigation, Stop the Bleed Training, public education programs, resource navigation. and expanded services for families with children. Also provides information regarding communicable diseases. Offers the following programs for expectant parents and new parents: HOPES Program, 1st Five, Early Head Start and the Maternal Child Health program. Immunizations for children, adolescents, and adults along with the flu vaccine.
Offers comprehensive health services and education, focusing on HIV, Hepatitis C, Gonorrhea, and Chlamydia counseling and testing, alongside overdose prevention with Nasal Narcan, and the distribution and training for Fentanyl and Xylazine test strips. Also offers blood pressure, glucose, and cholesterol screenings, Risk Reduction Counseling with safer sex supplies, and Harm Reduction Counseling with safer use supplies. Additionally, the program provides Covid home test kits, face masks, hand sanitizers, hygiene kits, and referrals to improve quality of life and curb the spread of HIV and other communicable diseases.
