Provides comprehensive support services for individuals and families facing mental health or behavioral challenges. Families build a personalized support team that may include professionals, service providers, friends, mentors, and other trusted individuals.
Provides intensive care coordination and wraparound services for Medicaid enrolled children under the age of 21 who have complex behavioral health needs and require intensive home and community-based services. There are two tiers of care coordination including:
-- Tier 1 – High Fidelity Wraparound for children and youth with the most complex behavioral health needs.
-- Tier 2 – Intensive Care Coordination for children and youth with complex behavioral health needs.
