Provides assistance with food, utilities, rent, medical expenses, and other immediate needs. Meets the client in their home and conducts a needs assessment. Provides referrals for community resources to support long-term sufficiency. NOTE: Saint Vincent de Paul provides general assistance services throughout Lake County. Individuals who do not meet residency requirements for this location will be referred to their nearest site.

Counseling Provides short-term counseling for individuals, couples, families, and children of active military. Offers up to 12 sessions (treatment may be extended under certain circumstances). Family Advocacy Program Handles reports of domestic violence, sexual assault, and child abuse involving military members. Provides intervention and treatment services including counseling, case management, and support groups.

Offers support groups for individuals who have lost a loved one to suicide. Offers eight-week groups on a rolling basis throughout the year. Offers a monthly in-person group in Lake County and online meetings for the following special interest groups: Spousal loss Parent support group Spanish speaking group STAR group for first responder families

Offers bilingual early childhood education program for parents and children under age five. Meets in-person two times per week. Also includes the following support services: Developmental screening Family planning education Parenting support Home visits by a trained support worker Support groups for grandparents raising grandchildren Referrals to health care providers, community services, and collaborating agencies

Administers Housing Choice Voucher program for income eligible households. Housing Choice Voucher program subsidizes part of the total rent. Housing Choice Voucher applicants are responsible for housing search and selection.

Offers hearing and vision screening conducted at preschools, private and public schools as requested. Children are provided referrals for further assessment as needed.

Provides services to individuals with severe disabilities so they can remain independent in their homes. Offers the following program options for independence:

• Personal Assistant (PA): Provides assistance with household tasks, personal care and, with permission of a doctor, certain health care procedures. PAs are selected, employed, and supervised by individual customers.

• Homemaker Services: Personal care provided by trained and professionally supervised personnel for customers who are unable to direct the services of a PA. Instruction and assistance in household management and self-care are also available.

• Maintenance Home Health: Services provided through a treatment plan prescribed by a physician or other health care professional. Other services include nursing care and physical, occupational, and speech therapy.

• Electronic Home Response: Emergency response system offered by hospitals and community service organizations. This rented signaling device provides 24-hour emergency coverage, permitting the individual to alert trained professionals at hospitals, fire departments, or police departments.

• Home Delivered Meals: Provided to individuals who can feed themselves but are unable to prepare food.

• Adult Day Care: The direct care and supervision of customers in a community-based setting to promote their social, physical, and emotional well-being.

• Assistive Equipment: Devices or equipment either purchased or rented to increase an individual's independence and capability to perform household and personal care tasks at home.

• Environmental Modification: Modifications in the home that help compensate for loss of ability, strength, mobility or sensation; increase safety in the home, and decrease dependence on direct assistance from others.

• Respite Services: Temporary care for adults and children with disabilities aimed at relieving stress to families. Respite services may be provided for vacation, rest, errands, family crisis or emergency. Services may include personal assistant, homemaker or home health.

Also provides specialized services for people with HIV/AIDS and/or traumatic brain injuries (TBI). The Community Reintegration Program helps individuals with disabilities who live in nursing homes move into community with the supports they need to live as independently as possible.

Offers a weekly food pantry for individuals and families in need. Provides fresh produce, meat, dairy products, shelf-stable foods, and feminine hygiene products. Also offers a monthly diaper distribution for pre-registered guests.

Offers a healthcare-based violence intervention program for survivors of violent crimes. Provides up to 16 treatment sessions to help survivors heal and overcome the effects of trauma. Comprehensive services include:

• Case management
• Individual and group therapy
• Psychiatry
• Medication management
• Support and advocacy groups
• Safety and self-care guidance

Offers parenting skills class for fathers to strengthen households and prevent domestic violence. Helps fathers to understand generational trauma, child development, positive discipline, and empathy.

Offers resources and services designed to meet the learning needs of the community. Also provides programs that introduce and encourage reading. Resources include:

• Reference services
• Computer and internet access
• Book delivery
• Notary (by appointment only)
• Voter registration
• Test proctoring

Offers pro-life pregnancy resources at no cost, which include: Pregnancy test with referral to County Health Clinic Diapers Maternity clothes Children's clothes up to size 10/12 Infant care items

Offers health screenings and treatment for minor conditions. Patients meet with a clinician over video while the in-person care team assists with the exam. Provides care for the following:

• Illness or injury
• Blood pressure screening
• Birth control and pregnancy testing
• Anxiety and depression screening
• STI services
• Asthma prevention
• Skin conditions

Provides supportive services for family caregivers of an enrolled veteran. Offers the following two programs based on level of care (see eligibility guidelines below): Program of General Caregiver Support Services (PGCSS)

• Individual support related to the care of the veteran
• Skills training on caregiving for a veteran's unique needs
• Respite care
• Peer support mentoring
• Training and support through in-person, online, and telehealth sessions
• Referrals to available resources

Comprehensive Assistance for Family Caregivers (PCAFC) - Provides all services listed above and the following:

• Mental health support
• Virtual Psychotherapy for Caregivers Program (VPPC)
• CHAMPVA (if uninsured)
• Monthly stipend
• Beneficiary travel

Offers food assistance for Warren Township residents once per month. Provides two grocery bags of non-perishable items and frozen meats to registered participants.

Offers supported-living group homes for adults with developmental delays. Develops a plan with an Aspire Life Consultant to learn independent living skills. Takes part in all aspects of running a home, such as cooking, cleaning, and grocery shopping.

Offers monthly financial support for township residents who cannot work due to health or medical condition. Provides support for rent, utilities, medical expenses, and other basic needs.

Offers a food pantry for individuals in need. Registered participants may visit up to two times per week.

Offers free preschool for children ages 2-5 years old. Prepares children and their families for success in kindergarten and later in life.

Offers affordable integrative healthcare for individuals of all ages. Outpatient health services include:

• Adult and senior health
• Infant, children and teen health
• Reproductive health, family planning, and prenatal care
• Dental care
• Behavioral health and counseling
• Substance use treatment services
• Medication-Assisted Treatment

A designated location that provides relief from the cold and heat for at-risk citizens. Center does not offer provisions or care. Offers relief from the extreme weather conditions only.

Serves as health center with variety of outpatient services. Provides Pediatric Services, Women's Health Services, Primary Care Services, and Dental Services. Pediatric Health Services: Provides complete care to children from birth to age 18. Services include: developmental assessments, hearing and dental screening, basic vision testing, immunizations, and lead testing. Women's Health Services: Provides a wide range of outpatient comprehensive services for women of all ages. Services include counseling, prenatal, and gynecological care. Primary Care Services: Offers a broad spectrum of personal and community services directed at individuals and families of all races, genders and ages who are unable to access the private medical sector. Dental Care Services: Provides primary dental care for children and adults, including: Dental Exams, Cleaning, Dentures, Digital X-rays, Fillings, Simple Extractions, Dental Sealants, Referrals to Orthodontics and Specialty Care, WIC Client Dental Screenings, School Dental Screenings, and Care for Pregnant Patients. (Payment for specialty care will be the responsibility of the patient.
