Esperanza Health Centers operates a number of Federally Qualified Health Centers (FQHCs) that are approved sites for immigration physicals, which are typically required for anyone who applies for lawful permanent resident status.

CareLink is a free program that helps Cook County residents cover the cost of care at any Cook County Health facility. CareLink discounts the cost of the following services: primary care visits, specialty doctor visits, clinic visits, lab tests, hospital stays and emergency services.Anyone living in Cook County may apply for financial help. They may also receive help applying for other state and federal medical programs. Patients must meet certain eligibility guidelines to qualify. Those who qualify for the program can receive care at Cook County Health with up to 100% discount on the cost of care.

The Chicago Community Health Response Corps offers an information and referral hotline service where community members can call to inquire about local resources related to COVID-19 vaccination and testing, housing assistance, food, medical needs, pet needs, and more. The Chicago Community Health Response Corps is not a direct service provider. Interpreter services are available to callers who speak a language other than English or Spanish.

Lemont Township operates the Vanpool transportation service to provide non-emergency transportation for Township residents to medical appointments that are located outside of Lemont Township. Vanpool is a door-to-door service and operates Monday to Friday, 8:00am to 5:00pm. Passengers requiring assistance (wheelchairs, etc.) must have a companion to accompany them. One ride-along passenger/caregiver travels at no charge.
