Provides services to individuals with severe disabilities so they can remain independent in their homes. Offers the following program options for independence:

• Personal Assistant (PA): Provides assistance with household tasks, personal care and, with permission of a doctor, certain health care procedures. PAs are selected, employed, and supervised by individual customers.

• Homemaker Services: Personal care provided by trained and professionally supervised personnel for customers who are unable to direct the services of a PA. Instruction and assistance in household management and self-care are also available.

• Maintenance Home Health: Services provided through a treatment plan prescribed by a physician or other health care professional. Other services include nursing care and physical, occupational, and speech therapy.

• Electronic Home Response: Emergency response system offered by hospitals and community service organizations. This rented signaling device provides 24-hour emergency coverage, permitting the individual to alert trained professionals at hospitals, fire departments, or police departments.

• Home Delivered Meals: Provided to individuals who can feed themselves but are unable to prepare food.

• Adult Day Care: The direct care and supervision of customers in a community-based setting to promote their social, physical, and emotional well-being.

• Assistive Equipment: Devices or equipment either purchased or rented to increase an individual's independence and capability to perform household and personal care tasks at home.

• Environmental Modification: Modifications in the home that help compensate for loss of ability, strength, mobility or sensation; increase safety in the home, and decrease dependence on direct assistance from others.

• Respite Services: Temporary care for adults and children with disabilities aimed at relieving stress to families. Respite services may be provided for vacation, rest, errands, family crisis or emergency. Services may include personal assistant, homemaker or home health.

Also provides specialized services for people with HIV/AIDS and/or traumatic brain injuries (TBI). The Community Reintegration Program helps individuals with disabilities who live in nursing homes move into community with the supports they need to live as independently as possible.

Offers fitness and recreation center for adults with intellectual or developmental disabilities. Includes the following:

• Special Olympics training
• Fitness center with equipment specifically designed for individuals with disabilities
• Pool with a wheelchair lift
• Movie theater
• Game room

Offers a confidential bi-monthly support group for adults living with a mental illness. The group is led by NAMI-trained facilitators who have been diagnosed with a mental illness, are well and recovered.

Provides law enforcement and emergency services within the City of Waukegan. Works with the community to promote safety, reduce crime, and preserve peace.

Provides an online tool to help consumers block illegal and unwanted calls. Helps prevent telephone scam calls.

Offers free mental health and wellness services to Township residents of all ages. Services include:

• Individual and Group Therapy
• Couple and Family Counseling
• Case Management
• Information and Referrals
• Lending Closet (free medical equipment borrowing program)

Offers grief support groups for those who have lost a loved one. Online search portal provides information for various in-person meetings throughout Lake County. Offers 13-week classes in-person and online.

Offers resources and services designed to meet the learning needs of the community. Also provides programs that introduce and encourage reading. Resources include:

• Reference services
• Computer and internet access
• Notary (by appointment only)
• Exam proctoring
• Bookmobile

Provides law enforcement and emergency services within the Village of Park City. Works with the community to promote safety, reduce crime, and preserve peace.

Satellite medical campus offering emergency care and outpatient medical services. The Ambulatory Care Center offers the following outpatient services:

• Physical, occupational, and speech therapy
• Imaging, MRI, and Mammography
• Laboratory services
• Sleep Center

Provides drug users a fast-track to addiction treatment services. Allows individuals to call or walk-in to Lake County Sheriff's Offices to request help. Individuals may surrender any drugs or paraphernalia without risk of criminal charges. A deputy will immediately coordinate phone screening with the Lake County Health Department. Based on the evaluation, individual will begin outpatient or inpatient treatment. Provides a police escort for any individuals who cannot transport themselves.

Hosts group gatherings for veterans to get together and establish a friendship. Provides free drinks and snacks.

Provides monthly financial assistance to help Township residents in need. Helps pay towards rent, utilities, or medical expenses. Provides temporary aide while individual finds a job or waits for benefits approval.

Offers weekly class for children, birth to three years of age, and their caregivers. Helps children develop social skills, practice cooperative play, and bond with their caregiver. Offers morning or afternoon session.

Offers free sewing classes for adults. Teaches participants how to use a sewing machine to make clothing and home products. Most items are donated to community members in need.

Provides a food pantry to residents of Vernon Hills, Indian Creek, and Mundelein. Also offers clothes, used furniture, and household items as they become available.

Provides monthly financial aid for township residents in need. Helps pay towards rent, utilities, or medical expenses. Intended for individuals who meet one of the following criteria:

• Cannot work due to medical condition
• Experiencing income or job loss due to circumstances outside of their control. Must be actively seeking employment.
• Have applied for government assistance and are waiting for approval

Provides monthly financial aid for township residents in need. Helps pay towards rent, utilities, or medical expenses. Intended for individuals who meet one of the following criteria:

• Cannot work due to medical condition
• Experiencing income or job loss due to circumstances outside of their control. Must be actively seeking employment.
• Have applied for government assistance and are waiting for approval

Provides financial counseling and coaching to help clients gain financial success. Conducts comprehensive analysis of clients' situation and develop an action plan. Addresses budgeting, increasing savings, decreasing debt, and improving credit scores. Provides follow up services to track progress and continue coaching, if needed.

Provides in home care for homebound seniors including personal care, dressing, bathing, grooming, meal preparation, medication reminders, light housekeeping, laundry, shopping, and errands.

Offers free adult learning classes to members of the public. Includes the following classes:

• English as a Second Language (ESL)
• Spanish GED classes
• Citizenship tutoring
• Spanish language classes
• Computer skills and resume writing

Offers monthly financial support for township residents who cannot work due to health or medical condition. Provides support for rent, utilities, medical expenses, and other basic needs.

Provides health care services for the LGBTQ population. Staff are trained and certified LGBTQ specialists. Offers all primary care services and the following specialty services: Hormone Replacement Therapy STI and HIV Testing and Treatment HIV Prevention Treatment (PrEP)
