Lawndale Christian Health Center (LCHC) provides primary care services to adults, including physical exams, vaccinations, anxiety/depression screenings, chronic disease management, and referrals.Please note that specialty care is only available at LCHC's Main Clinic (3860 W Ogden). Specialty care requires being an existing patient of LCHC and a referral from your primary care physician. Specialist services include cardiology, pulmonology, general surgery, orthopedics, ophthalmology, neonatology, radiology, ENT (ear, nose, and throat) and infectious diseases.

DFSS funds and oversees a network of overnight and interim housing shelter programs that includes over 3,000 shelter beds at 50 separate facilities operated by 29 different delegates. DFSS is integral to the coordination of shelter placement for Chicago residents. DFSS funds a delegate agency to respond to shelter requests around the clock. This includes transportation, triage and the placement of individuals and families in open shelter beds across the city. Additionally, the Shelter Placement and Resource Center (SPARC), located at 2241 South Halsted Street, offers a 24/7 in-person intake location dedicated to connecting single adults (not families) with shelter placements and providing interim basic services. The SPARC can accommodate up to 200 people awaiting shelter placement.

Leyden Family Services offers adult outpatient programs that include individual, marital, family, and group therapy. These programs are designed for people 18 and over who are having difficulty resolving inner conflicts and/or conflicts within the social groups of which they are members. No one will be denied access to service due to inability to pay.

Brite Dental provides regular dental care. Services include routine cleaning, teeth whitening, Invisalign treatment, tooth bonding, crown and bridge implants, tooth extractions, root canals, and deep cleanings.

Lutheran Social Services of Illinois (LSSI) offers services to individuals who have been arrested for "Driving Under the Influence" (DUI) or convicted of a similar offense. They accept Drug & Alcohol Evaluations completed by other organizations to help meet court requirements.

Health Promotion and Navigation promotes health and wellness by providing educational workshops year-round and assisting with screenings, referrals, navigation, interpretation, and transportation. Educational topics include cancer, cardiovascular disease, nutrition, osteoporosis, mental health, pre/post-natal care, nutrition, CPR, and sexually transmitted infections.

Impact Behavioral Health Partners provides stable, permanent, supportive housing and incorporates clinical and supported employment services to provide a well-rounded, holistic approach to the recovery of individuals with mental illness. Utilizing the "Housing First" methodology, their programs provide affordable housing to low-income individuals, many of whom are chronically homeless, and have chronic mental illnesses. They offer a stable foundation so residents can focus their energy on recovery, rather than where they will find shelter for the night.
