TCARE is the first tool of it's kind to assess the needs of the caregiver rather than focus on the care receiver. Through a series of questions, the TCARE assessment tool identifies the specific drivers of the caregiver's distress. With the specific stressors identified, an individualized care plan is created to help the caregiver feel more equipped to manage their role and identity as a caregiver. Interventions such as counseling, support groups and respite are put in place to manage stressors and prevent caregiver burn out.
