Provides mortgage counseling regarding budgets and other resources to avoid default. Also offers counseling on programs that will help clients save their home. All housing services are HUD-approved.

Provides sexual health services for Lake County residents ages 12 years and older. Provides comprehensive care and prevention services, including:

• HIV testing, counseling, and care
• Hepatitis C screening, counseling, and care
• Hepatitis A/B and HPV vaccinations
• STI testing, counseling, and treatment

• PrEP and nPEP medication

Offers clinical and faith-based counseling services for children, adolescents, and adults. All counseling sessions are via telehealth. Includes specialties such as marriage, bereavement, and abuse counseling.

Provides case management to help community members access primary care and behavioral health. Offers health education, community garden, Zumba classes, and referrals to community resources.

Provides door-to-door transportation for non-emergency medical appointments for older adults. Volunteer drivers transport clients to and from appointments using their personal vehicle. Vehicles can accommodate canes, light walkers, and small oxygen tanks. Provides a vendor transportation service for clients requiring a wheelchair. Also provides free grocery shopping service for seniors. Provides volunteers to shop and deliver groceries. The client must pay for the groceries.

Offers outpatient treatment for adults with opiate addiction. Includes detoxification and Medication-Assisted Treatment (MAT) services. Also offers methadone maintenance therapy to treat opiate use.

Offers independent living skills training for individuals with disabilities. Offers the following courses:

Provides general dental care for children and adults. Services include:

• Dental exams and routine care
• Electronic Dental Records
• Referrals to orthodontics

A designated location that provides relief from the cold and heat for at-risk citizens. Center does not offer provisions or care, it is for relief from the extreme weather conditions only.

Offers financial aid to help Township residents prevent loss of home, job, or utilities. Funds are available once every two years and provide help to meet basic needs, including:

• Preventing eviction or past due rent/mortgage
• Preventing utility disconnection
• Work-related expense necessary to getting or keeping a job (for example, uniform or licensing fees)
• Cost of prescription medication

Amount of assistance determined on a case-by-case basis. Program does not disburse cash as all funds will be paid directly to vendors.

Accepts furniture donations from individuals in the community. Distributes items to individuals in need for free. Notes: All pickups must be scheduled at least one week in advance. There is a suggested donation for pick-up service. All drop-offs require an initial call to ensure a quick unloading of items. This program does not provide free furniture directly to the public; must be referred by a caseworker to qualify.

Provides monthly financial aid for township residents in need. Helps pay towards rent, utilities, or medical expenses. Intended for individuals who meet one of the following criteria:

• Cannot work due to medical condition
• Experiencing income or job loss due to circumstances outside of their control. Must be actively seeking employment.
• Have applied for government assistance and are waiting for approval

**For emergencies, call 9-1-1** Provides fire protection, medical aid, and rescue. Offers hazardous materials response, and a variety of other services to the public.

Provides drug users a fast-track to addiction treatment services. Allows individuals to call or walk-in to Lake County Sheriff's Offices to request help. Individuals may surrender any drugs or paraphernalia without risk of criminal charges. A deputy will immediately coordinate phone screening with the Lake County Health Department. Based on the evaluation, individual will begin outpatient or inpatient treatment.

Operates 24/7 Adult Protective Services hotline to report adult abuse or neglect. Investigates reports of suspected neglect, abandonment, physical, sexual, mental, emotional, or financial abuse. Coordinates emergency response, crisis intervention, and protection from abuse, neglect, or exploitation.
