Dental Care Services: Provides primary dental care for children and adults, including: Dental Exams, Cleaning, Dentures, Digital X-rays, Fillings, Simple Extractions, Dental Sealants, Referrals to Orthodontics and Specialty Care, WIC Client Dental Screenings, School Dental Screenings, and Care for Pregnant Patients. Supported Living Arrangements (SLA) offers:

• Supports people with mild to moderate challenges
• Apartments where resident either lives alone or with a roommate
• Support with budgeting skills, grocery shopping or exploring work options to live as independently as possible

Community Integrated Living Arrangements (CILA) offers:

• Individuals to live as independently as possible in a home environment
• Staff to assist the residents with cooking, cleaning and participating in community activities
• Clinical (mental health and behavioral intervention)
• Recreation (community, campus, and home-based)
• Medical services provided to improve quality of life

Offers resources and services designed to meet the learning needs of the community. Also provides programs that introduce and encourage reading. Services include:

• Reference desk

Offers assistance filing for legal name change in the State of Illinois for LGBTQ+ individuals. Offers assistance with the following:

• Providing necessary court forms for the 19th Judicial Circuit Court
• E-filing court forms
• Providing name change and gender designation forms for the Illinois Department of Motor Vehicles, Social Security Administration, Illinois Department of Public Health and other states where available
• Obtaining certified copies of court orders related to name changes
• Alternate mailing address for court forms and notices

Offers the following transportation services for Waukegan Township seniors: Weekday Medical Transportation Provides curb to curb transportation for medical appointments within the township. Rides must be requested in advance. Shuttle runs weekdays only and is equipped with a wheelchair lift. Weekend Transportation Provides curb to curb transportation on weekends for travel within 10 miles. Rides must be requested in advance. Walking aids must be able to fold down for storage. Transit Card Issues a debit card to be used for personal transportation services. Funds can be used for any type of transportation service including taxis and ride shares.

Offers one-time financial assistance for rent or mortgage for Lake County residents. Clients may receive up to one month of rental assistance, once per year. Does NOT help with the following circumstances:
