Provides services to individuals with severe disabilities so they can remain independent in their homes. Offers the following program options for independence:

• Personal Assistant (PA): Provides assistance with household tasks, personal care and, with permission of a doctor, certain health care procedures. PAs are selected, employed, and supervised by individual customers.

• Homemaker Services: Personal care provided by trained and professionally supervised personnel for customers who are unable to direct the services of a PA. Instruction and assistance in household management and self-care are also available.

• Maintenance Home Health: Services provided through a treatment plan prescribed by a physician or other health care professional. Other services include nursing care and physical, occupational, and speech therapy.

• Electronic Home Response: Emergency response system offered by hospitals and community service organizations. This rented signaling device provides 24-hour emergency coverage, permitting the individual to alert trained professionals at hospitals, fire departments, or police departments.

• Home Delivered Meals: Provided to individuals who can feed themselves but are unable to prepare food.

• Adult Day Care: The direct care and supervision of customers in a community-based setting to promote their social, physical, and emotional well-being.

• Assistive Equipment: Devices or equipment either purchased or rented to increase an individual's independence and capability to perform household and personal care tasks at home.

• Environmental Modification: Modifications in the home that help compensate for loss of ability, strength, mobility or sensation; increase safety in the home, and decrease dependence on direct assistance from others.

• Respite Services: Temporary care for adults and children with disabilities aimed at relieving stress to families. Respite services may be provided for vacation, rest, errands, family crisis or emergency. Services may include personal assistant, homemaker or home health.

Also provides specialized services for people with HIV/AIDS and/or traumatic brain injuries (TBI). The Community Reintegration Program helps individuals with disabilities who live in nursing homes move into community with the supports they need to live as independently as possible.

Offers recovery coaching for individuals with substance use disorders. Helps in navigating recovery process and connecting to resources for recovery needs. Also provides online and in-person support groups for grief and recovery.

Provides comprehensive hospice care for adults who are near end of life. Offers care in nursing homes, hospitals, hospice facility, or in client's home.

Provides outpatient mental health services to youth and their families. Treatment Services include: Trauma-informed therapy for individuals, families, and groups, Crisis intervention, Substance abuse counseling, Case management, Psychiatric/medication services. Service Programs include: Screening Assessment and Support Services (SASS): A 90-day crisis intervention program, psychiatric evaluations and ongoing medication management; and case management. Treatment lasts about 3 to 6 months; Mental Health Juvenile Justice Program (MHJJ): Serves youth ages 8 to 18 years old; AND have had contact with the juvenile justice system within the last 90 days; OR are at risk of juvenile justice involvement; An MHJJ liaison that works with children and their families for six months and connects them to treatment resources; Intensive Placement Stabilization (IPS): serves youth who are in DCFS Youth in Care; AND are at risk of losing their current foster home placements; OR have just transitioned between placements; The Family Support Program (FSP): Serves children under 18 who need more intensive community-based therapeutic services or residential treatment services.

Provides curb-to-curb transportation to older adults and people with disabilities of West Deerfield Township. Priority given to medical appointments, but non-medical transportation may be available. Participants must be able to get to and from van without assistance of driver. Caregiver may ride with participant. Offers up to two round trips per week. Taxi-Cab Coupons: Provides taxi cab coupons for seniors and adults with disabilities. Coupons may be used for one way rides to and from Highland Park Hospital and are valued at $4.25.

Provides fire protection, medical aid, rescue, hazardous materials response, and a variety of other services to the public.

Offers $2000 reduction in a property's Equalized Assessed Value to persons with disabilities. Offered in combination with General Homestead Exemption and Senior Homestead Exemption. Cannot be combined with disabled veterans exemptions (veterans should choose the exemption that is most beneficial to them).

Coordinates educational services for students who are experiencing homelessness. Acts as a link between family, shelter, and school to serve the needs of the student.

Primary Care Services: Offers a broad spectrum of personal and community services directed at individuals and families of all races, genders and ages who are unable to access the private medical sector. Dental Care Services: Provides primary dental care for children and adults. Services include: dental exams, cleaning, and referrals to Orthodontics.

Provides transitional and permanent housing for victims of domestic violence or human trafficking. Provides counseling and other support services for all residents. Accommodates adults with or without children. Transitional Housing Provides affordable housing for survivors and their children for up to two years. Provides safety in a secure building. Scattered-Site Transitional Housing (Sanctuary of Hope) Provides safe, affordable housing and financial assistance for survivors and their families. Units are scattered throughout the Greater Chicagoland. Provides case management services to help reach educational, job, and financial goals. Permanent Supportive Housing Provides affordable housing within a 37-unit apartment building for survivors and their families.

Assists seniors through the process of qualifying for and obtaining a reverse mortgage loan (Home Equity Conversion Mortgage/HECM). Reverse mortgages allow homeowners to convert equity in their homes into income that can be used to pay for home improvements, medical costs, living expenses, or other expenses. Also offers housing counseling for seniors who are at risk of defaulting on an HECM loan.

Free summer school program for children entering kindergarten in the Round Lake District. Allows students to practice following directions and learn key skills. Program runs for three weeks during the summer and is taught by licensed teachers.

Enforces traffic laws and illegal drug and alcohol violations. Also provides certified child safety seat installation. Dial 9-1-1 for emergency assistance.

Provides 24-hour emergency relief to individuals and families affected by disasters such as house fires, earthquakes, floods, tornados, etc. Educates people in how to prevent, prepare for, and respond to emergencies. Health and Safety Services offer courses in first aid, CPR, child-care, nurse assistant training, babysitting, swimming-life guarding and more on a regular basis. Courses and training are offered at the Chapter facilities or can be arranged at different localities for businesses and groups. Blood Services collects blood donations and supplies safe blood and blood products to local healthcare providers. Also offers various types of volunteering opportunities for adults and youth.
