Operates a food pantry with the following items available:
- Nonperishable/canned foods.
- Frozen meats.
- Produce.
- Bread.
- Dairy products.
One visit per household allowed once per calendar month.
Provides case management on a long-term basis, up to one year or longer and as needed, to assist the family in overcoming the circumstances that brought them into an “at risk” position with homelessness, financial instability, less-than-living-wage employment, lack of education, substance abuse, mental health instability, physical health instability, legal issues, need for counseling, domestic violence issues, lack of child care, lack of transportation, and lack of basic household items, food, clothing and personal items to sustain appropriate health.
Assists individuals in Illinois in becoming licensed foster parents. Also provides follow-up training for foster parents to help meet the various needs of their foster children.
Services provided:
-- LCFS team provides guidance and support through the foster care experience.
-- Health insurance for the children placed in a foster home.
-- Therapy, counseling, and academic counseling, and special education services when needed.
-- Training programs, support groups, and newsletters geared to foster families.
-- Monthly financial support for the foster child(ren's) basic needs, including food, clothing, and housing costs.
-- Funding for daycare, after-school care, and extracurricular activities including sports and art programs for eligible children in foster care.
Provides a small group where teens can talk about their feelings, struggles, and achievements. A safe place where youth can be heard and know they are not alone.
Provides a mentoring program offering weekly discussions where youth can express themselves and share challenges they experience, fostering mutual support and encouragement. It also offers group outings with life skills themes, meals, and opportunities to gain new knowledge and skills while forming relationships with peers. The program emphasizes creating a supportive environment that addresses challenges related to personal and social development.
Offers mental health and substance use disorder services, including: recovery support and outpatient substance use treatment, DUI evaluation and risk education, psychiatric care, and outpatient mental health program.
Offers a combination of weekly support groups and parenting classes designed to strengthen the role of fathers and male caregivers, promote positive parenting, and prevent child abuse and neglect.
The 12-week support group series covers topics such as identity and purpose, self-care, and overcoming personal barriers, with options for in-person or virtual participation.
Parenting classes consist of an 8-session series focused on self-worth, child development, effective discipline, and creating safe, nurturing home environments.
All participants complete a comprehensive intake, and program staff provide referrals to community resources and case management as needed. Upon successful completion, participants in both services receive a certificate of completion.

Provides law enforcement services to the county, including responding to emergency assistance requests through a telephone response service staffed by trained personnel who assess situations and dispatch the appropriate emergency response agency.

- Alcohol and drug use, including opioids and heroin.

- Bipolar Disorder.

- Post-Traumatic Stress Disorder.
- Health issues related to diet, exercise, tobacco use, etc.
Counseling services also available at certain local schools.
Offers help with limited financial assistance (as funding allows) for basic needs such as rent, rent deposits, utilities, food, hygiene products, and more.
Clients must participate in ongoing case management.
Staff in each office are knowledgeable about services provided by other agencies and institutions in each area. They network within their communities to make sure they provide the best solutions for people in need and avoid duplicating services.
Patients in supportive care may receive visits or phone calls from a nurse practitioner, a nurse, or a social worker. Supportive care offers visits at FHN offices and at FHN Memorial Hospital, in long-term care facilities, and in the home for those unable to travel to the clinic due to physical disabilities or distressing symptoms.
